One of the roles of the Manchester Wellness Coalition is to analyze local health data and trends, and develop interventions to improve our overall health and wellness. We now have an opportunity to participate in one of these local health data collection efforts. As a community partner of both the Wellness Coalition and the Chelsea Community Hospital, the CRC is asking you to take a few minutes and participate in the St. Joseph Mercy Chelsea Community Health Needs Assessment (CHNA) survey, and invite your family and friends to do the same. A broad cross section of people throughout our service area is being asked to take the survey to help better identify health needs and how to address them. The Affordable Care Act requires all non-profit hospitals to conduct a community health needs assessment every three years. Results of the 2012 CHNA are available on the hospital website (stjoeschelsea.org) under Community Benefit.
This survey will be open through October 17th. Results will be presented at local wellness coalition meetings October 20th – November 20th, and included in the final 2015 CHNA report.
